Qualifications
Current license and registration to practice as an Occupational Therapist in NYS
Bachelors or masters degree in Occupational Therapy from a program approved by the NY Department of Education, OR a graduate certificate in Occupational Therapy from a college or university whose program is registered with NYSED or accredited by a national accreditation agency.
Hourly rate: Starts at $50 an hour and is commensurate with experience.

Board Certified Behavior Analyst (BCBA)
At Pathfinder School, the Board Certified Behavior Analyst (BCBA) is responsible for conducting functional assessments and/or functional analysis (as needed), develops, implements, monitors, and evaluates behavior support plans. The BCBA will design supportive environments and conducts staff training to teach new skills and manage behaviors to maximize independence in activities of daily living.
The BCBA consults with parents, advocates, and Pathfinder staff on a variety of behavioral management issues and provides on-going behavioral intervention support in all settings. The BCBA provides staff training and behavior plan reviews to ensure proper implementation of these plans.
Qualifications:
Required: Master's degree in behavior analysis or similar
Certification in behavior analysis (BCBA) or in the process of obtaining certification

Quality and Incident Management Coordinator

Primary Responsibilities
- Foster a culture of compliance, ethical practice, and continuous improvement at all organizational levels.
- Assist the Chief Compliance Officer in maintaining a formal Quality Improvement Plan (QIP) in partnership with agency leadership and programs to analyze and monitor progress towards quality improvement goals.
- Ensures the agency maintains an effective Incident Management Program that complies with NYS OPWDD, NYS Education Department, and New Jersey reporting requirements
- Serve as chair for the Human Rights Committee, maintaining meeting minutes and ensuring all incident records are accurate and complete.
- Trains staff on incident reporting, resident rights, and other safety/quality assurance topics as required.

Registered Nurse (RN) Case Manager
As a Registered Nurse Case Manager at Pathfinder Village, you will be joining a comprehensive team of RN’s, LPN’s, Clinical Office Assistants, and a Clinical Coordinator working in the rewarding field of developmental disabilities. You will work closely with our residents, their families, and clinical teams to assist and support them in making their healthcare decisions; provide staff support and education for assisting residents with their changing needs across their lifespan; and manage acute and routine healthcare needs. You will collaborate with the established relationships that we have with our local healthcare providers. In this position, you will provide telephone triage along with dedicated office and on-site hours coordinated by the Chief Clinical Officer.
Responsibilities:
Carry a caseload consistent with OPWDD guidelines and specific individual needs
Assist with attending appointments and communicating appointment results to residents, families, and staff
Facilitate and maintain ongoing staff trainings and education regarding healthcare needs of the resident
Work collaboratively with other department members to manage medications for residents
Provide hands on care consistent with a home healthcare model
Provide telephone triage services and on call rotation as assigned by the Director of Health Services
Maintain CEU’s of approximately 6-10 per year
Qualifications:
Bachelor's degree (BA or BSN) from an accredited four-year college or university, OR
An Associate's degree (ASN) from an accredited two-year private vocational school or community college with related experience
Required Certifications:
NYS Registered Professional Nurse License
Valid NYS driver's license
Current certification in CPR

Vocational Support Professional

The role of the Vocational Services Professional (VSP) at Pathfinder Village is to deliver prevocational services to adults with I/DD who have expressed a desire to work and earn a paycheck. Vocational services not only provide individuals with opportunities to explore a variety of career paths and jobs, but also develop soft/hard skills that will prepare them for entering today's modern workforce. Working under the supervision of the Prevocational Site Supervisor, VSPs will have opportunities to work in a variety of setting both on and off campus, as well as network with our various community business partners.

Plan Writer
Pathfinder Village is seeking a qualified Plan Writer in our compliance department. The Plan Writer is responsible for the development and coordination of formal supports and services for individuals with disabilities receiving applicable services at Pathfinder Village. Upholding individual rights and promoting person-centered outcomes while ensuring personal safety are essential hallmarks of this position. As a member of an individual's interdisciplinary team, the Plan Writer writes formal habilitation plans, trains staff, attends meetings, ensures regulatory compliance, and acts as a liaison with care managers. If necessary, the Plan Writer will participate in meetings to address behavior and medical needs.
Required Qualifications:
Bachelor’s degree in the human services field
Occasional travel is required, valid NYS driver's license is required.
Excellent verbal and written communication is required.
Proficiency or knowledge of using a variety of computer software and email applications, especially Microsoft products.
Preferred Qualification:
1 year of experience working with individuals with disabilities.

Community Habilitation Specialist
The Community Habilitation Specialist provides support in a home or community setting that promotes success at the highest level of independence possible In addition to program-specific functions, this will include, but not be limited to: teaching and reinforcing a wide range of daily living skills, promoting independence, preventing and responding to crisis situations appropriately, encouraging self-advocacy and facilitating the development of natural supports. All services will be provided in collaboration with program participants and their service plan, utilizing a person-centered approach.

Special Education Teacher

This position offers a competitive salary, with certified teachers starting at $58,000 for the 10-month school year. Those who choose to work the summer session can earn an additional $9,600, bringing the total potential earnings to $67,600. Candidates who are actively working toward their certification will be considered at a lower starting rate.
Responsibilities include developing and implementing individualized education plans (IEPs), providing differentiated instruction to meet the unique needs of each student, and fostering a positive and engaging classroom environment. Direct Support Professional (DSP)
Pathfinder Village presently has 12-hour shift openings for Direct Support Professionals. Primary responsibilities include providing direct care and residential guidance/supports for individuals living with intellectual and other disabilities within an open-access community setting. Additional duties involve advocating and supporting informed choices of individuals within the greater community. Strong record keeping skills, local travel, flexibility, and a commitment to ongoing job training are required.
- 3 - 4 day work week.
- No prior work experience is required.
Starting hourly rate ranges from $16.25 to $20.50 an hour, commensurate with experience. Overnight staff get an additional $3 an hour shift differential on top of their base rate when working an overnight shift.
