Day Services

Pathfinder Village provides community services within a 20-mile radius to Edmeston, New York. We primarily serve adults living in northern Chenango, Delaware, and Otsego counties, and southern Herkimer, Madison, and Oneida counties.

Our innovative community-based services are designed to support people of all abilities in becoming active and contributing members of their communities. Using highly individualized approaches through person-centered planning:

  • We discover what is most meaningful to a person.
  • We listen to what they want out of life.
  • We learn about their personal preferences.
  • We seek to understand any barriers or challenges they may experience.

Our professional and experienced team can create a customized action plan to help people achieve their personal goals by utilizing one or more of our community-based services.

Supported Employment

Our Career Center works closely with individuals who are ready for competitive employment in the community. We serve as a resource to help people navigate the ACCESS-VR process, create resumes, sharpen their interview skills, complete job applications, etc. We can also network with our various community partners to help create a “job match” that is compatible with an individual’s goals, desires, and abilities. If an individual is approved for formal Supported Employment services through OPWDD, we can also provide a specially trained Job Coach to offer on-going support to ensure an individual remains successful on the job.

Employment Training Program (ETP Internship)

This unique program works in partnership with local business partners and OPWDD to provide paid, one-year internship opportunities. The business agrees to a trial period and OPWDD pays the initial wages. Our supported employment staff assists the intern throughout the program as needed. After one year, the expectation is that they will have the training and skills to become a permanent employee of the business. This program requires successful completion of a comprehensive application and discovery process before individuals are selected for paid internships.

Community Prevocational Support

Community-Based Pre-voc services are designed to assist individuals who have expressed an interest in career exploration and/or building their pre-employment skills in the hopes of one day obtaining gainful employment. In this program, a specially trained Pre-Voc staff member facilitates a structured learning curriculum as well as exposes the individual to a variety of real-world, community-based job experiences at local businesses within our community.

Community-Based Day Habilitation “Without Walls”

“Without walls” programs are day hab services that offer a higher degree of community participation, exploration, and volunteering than traditional site-based day hab programs. It can also provide an entry-level pathway to other adult pre-vocational and employment training programs in the future. Pathfinder Village offers a few options within this model to accommodate a wide range of interests. We facilitate trips to local museums, libraries, and fitness centers; attend enrichment classes such as art, music, and Tai Chi; and participate in various outdoor activities and events.

Community Habilitation

Community Habilitation services focus on teaching the skills needed to help a person become as independent as possible at home and in the community. Areas of focus may include completing household skills such as grocery shopping, maintaining a clean home, meal planning, and budgeting as well as community-based skills such as utilizing public transportation and community safety. Services are available for people who live independently, with family, or in a certified residence.

Self-Directed Services

Self-Direction gives you the option to choose your own services and supports so you can live the life you want. By self-directing your services, you have the flexibility to choose the staff you want to work with, the activities you want to participate in, and many other supports and services you choose to access.
