Transition Specialist

Pathfinder Village is seeking a Transition Support Professional (TSP). The role of the TSP is to promote a supportive and nurturing environment for the Otsego Academy students leading to lives of greater independence. His/her focus will ensure the safety, health, well-being, and satisfaction of each student. The TSP will work closely with the Otsego Academy Instruction Specialist to support instruction in the areas of academics, vocational preparation, and independent living. The TSP and the Otsego Academy Instruction Specialist will work together to provide meaningful instruction to the students and the TSP will be sure the instruction is continued into the evenings.

A high school or equivalency diploma is required, and experience in the OPWDD field preferred. This position requires a valid NYS Driver’s License.

School Nurse

Pathfinder Village is seeking a qualified School Nurse at Pathfinder School, a private day/residential school for students with Down syndrome, autism, and other intellectual and developmental disabilities. The School Nurse will provide primary health and emergency care to students, serve as a resource for health concerns in the school setting. Additional responsibilities include assisting with education as needed for Pathfinder Village.

Applicants must hold a current New York State license as a registered nurse and a current CPR certification. Preferred qualifications include a bachelor's degree in nursing or related field, experience in pediatric, community, and public health nursing, and previous experience in school nursing.

Classroom Aide

Pathfinder Village, a dynamic residential community for children and adults with Down syndrome and other intellectual disabilities, is seeking the following position in accordance with the Pathfinder Village mission “that each life may find meaning.” The Classroom Aide supports students to become more independent in the areas of academics, communication, behavior management, pre-vocational training, and other areas of daily living skills. We are currently accepting applications for full-time classroom aides.

Hours are Monday – Friday, 7:00am -3:00pm. A high school or equivalency diploma is required, and experience in the OPWDD field preferred.  This position requires a valid NYS Driver’s License. Salary range and benefit package are regionally competitive. 

Community Hab Specialist

Pathfinder Village is seeking a Community Hab Specialist. A Community Hab Specialist’s focus is to ensure the safety, well-being, and satisfaction of each individual enrolled and assist with day to day tasks based on individuals needs when living on their own, often for the first time. This can include teaching basic hygiene skills and management, establishing schedules/routines, teaching cooking skills, providing transportation, assisting with shopping, and maintaining a clean living environment. In this setting the Community Hab Specialist also works with highly confidential information such as banking, medications, hospital records etc.  Detailed documentation is required to capture services provided.

A high school or equivalency diploma is required, and experience in the OPWDD field preferred. This position requires a valid NYS Driver’s License.

Adult Day Services Worker, Full-Time

Responsibilities include providing formal and informal training on all habilitation and vocational activities to individuals served by Pathfinder Village Day Services, and helping the individuals develop and maintain skills. Adult Day Services Specialists must also maintain positive community relationships at all community and activity sites.

Hours will be 7:30am – 3:30pm, Monday - Friday, and as assigned by the Adult Day Services Manager. At least one year experience working with individuals with developmental disabilities is required, and a clean NY State driver’s license (with at least 3 years of driving experience). Experience working in a Day Services setting is preferred. A high school or equivalency diploma and computer skills for various software programs is necessary.

Direct Support Professionals (Overnights and Day Shifts)

Pathfinder Village presently has 12-hour Overnight and Day shift openings for Direct Support Professionals. Primary responsibilities include providing direct care and residential guidance/supports for individuals living with intellectual and other disabilities within an open-access community setting. Additional duties involve advocating and supporting informed choices of individuals within the greater community. Strong record keeping skills, local travel, flexibility, and a commitment to ongoing job training are required.

No prior work experience is required. Competitive wages and excellent benefits are offered.
