Service Coordination

Pathfinder Village service coordinators perform a key service that is separate and distinct from other services that a person with a disability may receive. Service coordinators help people with disabilities live successful lives according to their dreams, desires and individual plans.

Working in partnership with each person, the service coordinator’s role is to assist him/her to achieve a balanced and integrated life perspective, always mindful of safeguards and protections that are necessary in developing an individual service plan.

Pathfinder Village service coordinators link individuals to those services and supports that will enable them to experience the quality of life that they desire. These may include educational services, day services, employment, residential services, volunteer experiences, and other activities.     

Quality Assurance

Oversight of standards and regulations from 10 different state agencies comprise the external quality monitoring entities. 360-degree internal quality review includes monitoring state agency protocol for compliance as well as Pathfinder Village’s expectations for quality care and oversight.

Every part of the organization is thoroughly reviewed by our objective quality assurance staff. Results of this comprehensive audit are reviewed by the agency Corporate Compliance Committee and the Operating Board of Directors.

Our proactive and transparent approach to quality assurance gives each family and service participant peace of mind that safety and service integrity meet the highest possible standards.
