Transition Specialist, Otsego Academy

Pathfinder Village, a dynamic residential community for individuals with Down syndrome and other intellectual disabilities, is accepting applications for a Transition Specialist for Otsego Academy, a post-secondary program for young adults. The role of the Transition Specialist is to promote a supportive environment in which young adults with intellectual disabilities can develop greater independence in the areas of academics, vocational preparation, and independent living. The Transition Specialist will maintain positive relationships with Pathfinder Village’s college and business partners. This is a full- time position, M-F, 7:00am-3:00pm.

A clean NYS drivers’ license with at least 3 years driving experience is required. A high school or equivalency diploma is required, and an associate’s degree is preferred. First Aid/CPR, Medication Administration certification, and effective communication skills are also necessary. Pathfinder Village offers a competitive salary with an excellent benefits package. Pathfinder Adult Day Services Specialists:

Pathfinder Village is currently accepting applications for Full-Time Adult Day Services Specialists. Responsibilities include providing formal and informal training for all habilitation and vocational activities to individuals served by Pathfinder Village Day Services. ADS staff also help individuals develop and maintain skill levels.  Adult Day Services Specialists must also maintain positive community relationships at all community/activity sites. Hours will be 7:30 a.m. to 3:30 p.m., Monday-Friday, and as assigned by the Adult Day Services Manager.

Additional ADS staff are being recruited for part-time and per diem schedules, with hours as assigned by the Adult Day Services Manager.

At least one year experience working with individuals with developmental disabilities, and a clean NY State driver’s license (with at least three years of driving experience) is required. Experience working in a Day Services setting is preferred.

A high school or equivalency diploma, and computer skills for various software programs is necessary. Pathfinder Village offers a competitive salary with an excellent benefits package. Direct Support Professionals, Full-Time, Day Shifts:

Pathfinder Village presently has two full-time openings for Direct Support Professionals, offering daytime (7 a.m. to 3 p.m., Mon. - Fri. schedules). Primary responsibilities include providing direct care and residential guidance/supports for individuals living with intellectual and other disabilities within an open-access community setting. Additional duties involve advocating and supporting informed choices of individuals within the greater community. Strong record keeping skills, local travel, flexibility, and a commitment to ongoing job training are required. No prior work experience is required. Competitive wages and excellent benefits are offered.

Pathfinder Village presently has full-time openings for Direct Support Professionals, offering overnight (11 p.m. to 9 a.m. schedules. Primary responsibilities include providing direct care and residential guidance/supports for individuals living with intellectual and other disabilities within an open-access community setting. Additional duties involve advocating and supporting informed choices of individuals within the greater community. Strong record keeping skills, local travel, flexibility, and a commitment to ongoing job training are required. No prior work experience is required. Competitive wages and excellent benefits are offered.
 

Direct Support Professionals,
Full-Time, Evening Shifts:

Pathfinder Village presently has several full-time openings for Direct Support Professionals, offering evening (3 p.m. - 11 p.m.) schedules. Primary responsibilities include providing direct care and residential guidance/supports for individuals living with intellectual and other disabilities within an open-access community setting. Additional duties involve advocating and supporting informed choices of individuals within the greater community. Strong record keeping skills, local travel, flexibility, and a commitment to ongoing job training are required. No prior work experience is required. Competitive wages and excellent benefits are offered. 
 

Direct Support Professionals,
Part-Time & Per Diem Schedules:

Pathfinder Village presently has part-time and per diem openings for Direct Support Professionals, offering a variety of hours (8 a.m. to 3 p.m.; 3 p.m. to 11 p.m. or 11 p.m. to 9 a.m.) and environments. Primary responsibilities include providing direct care and residential guidance/supports for individuals living with intellectual and other disabilities within an open-access community setting. Additional duties involve advocating and supporting informed choices of individuals within the greater community. Strong record keeping skills, local travel, flexibility, and a commitment to ongoing job training are required. No prior work experience is required.
